You will be responsible for completing a respiratory assessment on your classmate related to the case study described below Case study The nurse is caring for a patient who is coughing up greenish-yellow sputum. The patient is complaining of shortness of breath. The nurse found the patient to have decreased breath sounds on the right side Subjective data: (what types of questions would you ask?) Objective data: (what would you assess?) Additional questions: How would the nurse position the patient to perform this assessment? Due to the decreased breath sounds on the right, the nurse should perform what additional aspects of the respiratory assessment? When percussing the patient's chest, what sounds should the nurse expect to document? What is an appropriate nursing diagnosis for this patient?   • color of sputum.
  • consistency of cough
  • ask about how she/he is feeling.
  • ask about blood is coming in sputum or not.

objective data-

  • we will assess the vital signs of patient mainly temperature and respiratory rate.
  • we will assess the color of sputum
  • send the sputum for sputum culture in laboratory.
  • assess the weight of patient daily.
  • assess patient is taking their diet properly or not.
  • assess the patient for peripheral edema
  • assess the saturation level of patient.
