Answer #1

#. Critical thinking

involves recognizing that an issue exists (patient problem), analyzing information about issues (clinical data about a patient), evaluating information (reviewing assumptions & evidence), and making conclusions.

#. Clinical reasoning

- critical thinking in a clinical setting

- process by which nurse reaches a clinical judgement

- process of noticing, interpreting, and responding

- reasoning in transition with a fine attunement to the patient and how the patient responds to the nurse's actions

#. Clinical judgement

- interpretations and inferences that influence actions in clinical practice

- an interpretation or conclusion about a patient's needs, concerns, or health problems, and/or the decision to take action (or not), use or modify standard approaches, or improvise new ones as deemed appropriate by the patient's response

#. Tanner's Model of Clinical Judgement

1. noticing = a nurse notices things about a patient in the context of the nurse's background, his/her experience, the environment, and the patient; a nurse is looking for patterns that are consistent with previous experiences and uses that information to guide care

2. interpreting = process of assembling information to make sense of it; varies with the experience of the nurse; novice nurse rely on analytical reasoning, while expert nurses draw from a variety of reasoning patterns: analytic, intuitive, narrative

3. responding = the implementation of actions and interventions, based on the patient's needs

4. reflecting = process of thinking and learning from experiences; reflection in action happens in real time while care is occurring
