Nursing intervention and rationale

• Assess the level of pain,intensity, duration, frequencies, type to get the baseline data and plan for care
• Administer medication as per order to relieve pain
• Provide comfort devices and measures to ease the patient
• Provide diversional therapy to make patient feel psychologically strong to overcome pain
• Provide relaxation therapy like meditation, massage,music to soothe the patient and relieve tension
• Avoid triggers that stimulate pain sensations in patients.
• Inform provider when pain doesn't settles down
