• Measure: Measuring the length, depth and the dimension of the wound.
• Exudate: It includes the assessment of the wound regarding wound fluid such as cellular debris, serum etc. Also inspecting the amount such as small, large or the medium wound size. Presence of any color such as red, yellow, black in color.
• Appearance: The appearance of the wound that is the eschar is brown or black avascular tissue. Checking the adherence as it may tightly adhere or loosely adhere to the wound.
• Sufferings: The intensity of the pain and area affected or suffered by the patients.
• Re-evaluate: Re-evaluating the wound such as any kind of injury in feet, or diabetic conditions etc.
• Edge: A condition occurs that extends four centimeters from the edge of the wound.
