How long do you have alcohol and smoking habits?
3, Multiple choice questions like selecting a few possible choices
How often you eat junk food?

When you complete the survey you need to analyze and compare the highest proportions of similar responses. Share the survey result with coworkers and proceed with another survey if it is needed. It provides results used by the organization what kind of service they needed.
