1. Description of the skill : when mixing two types of insulin always remember first to put clear insulin which is regular insulin and then use cloudy insulin.

  • Check for dose of insulin prescribed by the physician.
  • Wash your hands and wear gloves
  • Roll the vial of cloudy insulin between your palms before drawing insulin from it to mix the insulin well.
  • Clean the top of vial with alcohol swab
  • Inject the same amount of air in both the vials one by one equal to the units of each insulin prescribed.
  • Now turn the vial of clear insulin upside down and draw exactly the same amount of insulin as prescribed.
  • Now draw insulin from tge cloudy vial. Check the reading on the syringe to confirm the amount of insulin.
  • Administer the injection to the patient.

2. Insulin is indicated for the patients suffering from diabetes. Insulin therapy is used in insulin dependent diabetes as well as non insulin dependent diabetes.

3. Nursing interventions : includes monitoring the blood sugar, educating the patient for signs of hypoglycemia, inspecting the injection site for infection, swelling, pain, emphasise on healthy diet.

4. The outcome of insulin administration is controlling the blood sugar levels for diabetics. Evaluate the patient for signs of hypoglycemia.

5. Client education includes

- always draw clear insulin before the cloudy

- keep rotating the site of injection

- do not shake insulin vials. Roll the vial between the palms of hand.

- educate patient about signs of hypoglycemia like dizziness, confusion, increased perspiration, drowsiness, weakness

- emphasise on healthy diet with low sugar content

6. Potential complications include sudden hypoglycemia, infection of injection site.

7. Nursing intervention for complications include regular monitoring of blood glucose levels. If the patient already have low blood sugar then skip or lower insulin dose.

Keep rotating the site of insulin injection. Rub the injection site after injection administration. Use cold packs.
