DRUG: Morphine

CLASS: Opioid agonists

ONSET:10-30 min

PEAK: 1-4 hours

DURATION:5 hours upto 24 hours

(Onset,peak and duration may change according to the route of administration.But the above mentioned is for overall administration).

INDICATION: Severe Pain and palliative care

ACTION: Binds to pain receptors alter the pain perception thus reduce the level of pain and produce CNS depression in some patients.

THERAPEUTIC EFFECTS: Reduce the pain severity

For Adults:

PO- 30 mg,IM/IV/SC- 4-10 mg and epidural - 5mg/day.

For Children:

PO-0.3-0.6mg/kg Q​​​​​​​​​​​12 hr.

IM/IV/SC-0.05-0.2 mg/kg Q​​​​​​4 hr.

Epidural -0.1mg/kg/24hr

ADMINISTRATION:

It depends on the need of the emergence the dose and route of administration varies.

CONTRAINDICATION:

Hypersensitivity,Mild pain and Respiratory depression.

ADVERSE /SIDE EFFECTS:

MOST COMMON: confusion,sedation,hypotension and constipation

LIFE THREATENING: Respiratory depression.

INTERACTIONS: Morphine- Warfarin - increase the anticoagulant effect.

Along with anti-psychotics increase the side effects also leads to Respiratory depression.

NURSING CONSIDERATIONS:

Assess the conscious level,BP,pulse and respirations.

Assess the level of sedation

Assess the risk for drug addiction.

PATIENT EDUCATION:

Instruct about when to use the drug.

It alters the conscious level hence advice patient to avoid the conscious activities such as driving

Advice to do Respiratory exercises to prevent the Respiratory problems like Atelectasis.

EVALUATION/DESIRED OUTCOME:

Decrease in severity of pain without causing any adverse effects.
