For any disorder to identify screening is very essential, the three screening strategies are as follows:

SBIRT(Screening,Brief intervention and Referral to treatment) recommend two main strategy which are the standard approach and quick approach.

The standard approach involves 2step

  • STEP1-Ask about alcohol and drug use(
  • STEP2A-If AUDIT C positive,assess for alcohol use severity.
  • STEP2B-If single item drug screen positive assess fir drug use severity

The quick approach involves 3step

  • STEP1:Ask about alcohol and drug use
  • STEP2:Assess for alcohol drug severity
  • STEP3:Brief intervention -Understand patients view of use,Give information/ feedback,Enhance motivation to change,Give advice and negotiate goal.

The other strategies that can be used for screening are brief assessment to categorize patuent into 3 categories hazardous use,substance use or substance dependence .

There are various evidence based practices tools like Assessment and data  collection

Use of various scales and approaches to evaluate the substance use. The three strategies to engage patients to  discuss their substance use are:

Maintaining therapeutic milleau

Peer group duscussion where we engage the patient in Alcohol Anonymous group to discuss among themselves about the use of alcohol,its effects and problems that occurs after consumption of alcohol.

Cognitive Behavior Techniques to be used to know the patients exact attitude.

  • The conversation can be started by greeting the patient and regards for his health.Provide initially assurance that the information provided would be mainatained confidential in order to gain the trust of patient.The discussion about SUD can be beginned with a casual talk and there by with the use of  AUDIT scale (in alcohol use)data can be gathered.

The main concerns that are to be addressed are:

  • The clients and his/ her family wellness.
  • The duration and the cause for its use.
  • The impact of substance use for the client and his family members
  • The financial burden faced due to substance use
  • The emotional and social stigma faced by substance use patients.

Yes there are certain values/attitude that comes up after hearing this story which say that any substance when becomes addicted that changes the life of the individual and the family members.Addiction of the person is than transformed into dependency where without it the patient cant live, to this situation the client feels.

Countertransferance is the unconsious response of the therapist towards the patient.Thus it can be managed by

  • Empathy
  • Self insight
  • Conceptual ability
  • High therapist self integration
  • Low therapist anxiety

Cognitive Behaviour therapy is one of the important intervention that can be applied ti patient where the maladaptive behavior js tranformed into adaptive behavior.This therapy helps the patients treatment in an positive ways like Reinforcement,Token economy and involvement of patient for ones treatment.
