Baby Mitchell, 2 months old, is brought to the hospital by his mother. His mother states "It started this morning, he just gets so tired when he eats, and sometimes when he is nursing he has bluish color around his lips." Mitchell is fussing during triage.

1. What concerns you with the above scenario? (what could be going on-- differential diagnoses? which systems are you concerned with? what part of the story from mom is significant)

Mitchell is diagnosed by EKG with SVT (supraventricular tachycardia).

2. What might you anticipate next? (what would you do next, or what my the doctor order next?)

Answer #1

The child is just 2 months old infant. The mother told child got weak after feed, and usually cyanotic spell occur after a task like eating, playing etc.

The mother was telling about blue coloration around the mouth. Its mentioning the Central cyanosis , which has some specific causes.

It can be:

  • Lung problem or breathing issues
  • Congenital heart diseases or defects
  • Problems with blood flow to and from lungs. Any abnormal connections in blood vessels can be there.
  • Pulmonary edema caused by congestive heart failure.
  • Defects of hemoglobin, causing oxygen deficiency in the system.

How to manage Mitchell?

The baby will be managed for arrhythmia first, because SVT can lead to lethal health condition in the baby.

Anti arrhythmics may be used to manage the SVT.

Close monitoring of the child is done while on anti arrhythmics, to prevent chances of bradycardia.

A thorough antenatal as well as health history of the baby will be taken from mother.

Once the baby is stabilized, the doctor will order for diagnosis of underlying disease condition.

The baby may be ordered chest Xray, echocardiogram and blood test to have a detailed look into the health condition.
