Why monitor kidney function because vancomycin can affect renal function. also suggests that the patient also be monitored for nephrotoxicity and ototoxicity, which 2 additional lab tests would help determine this? What nursing intervention could you consider adding to help prevent nephrotoxicity?

Answer - vancomycin is a glycopeptide antibiotic used to treat methicillin resistant staph aureus. It is primarily excreted by the Renal route.

We have to monitor the trough level regularly to look out for nephrotoxicity. If level is greater than 15 MG, there is chance of of Acute kidney injury. Avoid using vancomycin with other nephrotoxic drugs like diuretics, aminoglycosides, acyclovir, amphoterecin b, etc. If acute kidney failure occurs, go for haemodialysis.
