Interprofessional team refers to the coordination of two or more profession working together to achieve a common goal or objectives. They work together to achieve simple to complex issues.In health care inter professional team includes doctors, nurses, pharmacists, physician assistants, dentists, Laboratory technicians, rehabilitation services,dietician,skilled assistants.

Inter-professional team for evidence base practice or quality improvement project and why this team would be best suited to address this issues concerning medical errors:

- Evidence based practice refers to the integration of research findings into patient care. The idea inter professional team for EVBP is doctors and nurses team. Medication error is defined as prescribing/ administering wrong drug in a wrong dosage that may potentially lead to harmful effects.

- EHR act as a tool to prevent medication errors. Mutual understanding and cooperation between nurses and doctors facilitates effective usage of EHR in preventing medication error.

The components of EHR includes :

a) Patient Data : Unique ID, demographics, insurance claimed, contact details.

b) Clinical data : medical records, nursing charts, medication records, intake and output chart

EHR provides all baseline data about patient and it can be access anytime.

2) EHR can be stored easily .

3) Entering EMR code of the patient will help us to locate patient data without wasting of time.

4) EHR encoded with Electronic medical record ease the drug administration and reduces medication errors.

Doctors must enter all the patient data in EHR. Prescriptions can be given as e-prescription, when the nurse practitioner about to administer medication for a patient scanned bar code on the wrist band of the patient gain access to digital medication slip. All 7 rights of medication is checked using digital prescription. This helps nurses to avoid medication errors like overdosing, wrong patient, wrong site, adverse reactions of drugs.
