43. An older male patient has trouble urinating and never feels as if he has voided his bladder. The diagnosis is BPH.

He advised tamsulosin(Flomax)

a. tamsulosin is an alpha 1 antagonist. It relaxes the smooth muscles which tend to relax blood vessels.

b. It does not act by shrinking prostate rather it relaxes muscle near the bladder neck(detrusor) and muscle near the prostate.

c. Decreased libido and abnormal ejaculation are side effects of tamsulosin, these symptoms seen with approximately 4 to 5% of patient
