Outcome measurement is usually the concept that is well followed during the patient's stay in hospital as it is a patient centered process and addresses the clinet's requirements well.

There is a common notion that outcome management is onyl a part of hospital services, this is false, as outcome management should also be the part of post-discharge services. This helps in maintaing and following up on the treatment efficacy post-discharge.

Therefore, outcome management should not end at the discharge of the client.
