2. Identify 1 (one) specific measurable patient learning need. What is it that the patient needs to know or needs to be able to do? Be sure to be specific.
3. Identify 3 (three) measurable, behavioral objectives for the teaching plan.
   1. Determine what you want the patient to learn.
   2. Use measurable verbs such as list, define, explain, state, describe, demonstrate, summarize, etc.
4. Develop the teaching plan tailored to the needs of the patient selected.
   1. Create a Concept Map for teaching plan.
   2. The nursing diagnosis Knowledge Deficit is recommended, but is not mandatory. Other nursing diagnoses may be used. Be creative.
5. Evaluate patient learning.
   1. Create a way to measure the patient’s learning.
   2. Determine if he/she has learned the material or have patient demonstrate the skills taught.
6. Present your plan and evaluation in post clinical conference.
