please help me with the 3 priority nursing diagnosis, goals and intervention/rationale for the patient with "Renal cancer" in this question below.

Dudley Wayne is a 62-year-old factory worker. Mr. Wayne makes an appointment

with his primary care practitioner because he has lost 15 pounds in the last 2 months, and

has recently noticed blood in his urine. He denies pain on urination. During the admission

assessment, Mr. Wayne states that he is a two-pack-per-day smoker and just thought he

was losing weight because of increased work stress. He also states that he worked 30

years in a building that was recently closed due to asbestosis contamination. Upon

physical examination, Mr. Wayne complains of pain when the provider percusses and

palpates his flank area. A CT scan is ordered along with an ultrasonogram; both reveal a

mass in the renal pelvis.

1.Nursing Diagnosis

Acute pain (flank area) related to cancer of kidney as evidenced by renal mass

Goal/outcome

To relieve pain

Nursing intervention and rationale

  • Assess the pain level,intensity, duration, frequency, type to get the baseline data and plan for care
  • Administer analgesics as per the order to relieve pain
  • Prepare the patient for procedure (chemotherapy, radiation, excision ) to remove the cause of pain
  • Provide comfort devices to ease the patient
  • Provide diversional therapy ,so that the patient feels little better psychologically
  • Follow the advice of pain therapist if any ,to provide a greater relief

2.Impaired urinary elimination related to renal cancer as evidenced by blood in urine secondary to smoking

Goal/outcome

To relieve the payient from passing blood in urine

Nursing intervention and rationale

  • Monitor patients intake and output ,to prevent any major complications
  • Administer medication to stop hematuria
  • perform urine test as per order to rule out the RBC in it and provide care
  • Administer fluid as per order ,to clear the urinary tract
  • Encourage the patient not to strain while voiding

3..Imbalanced nutrition less than boad requirement related to the disease condition as evidenced by weight loss

Goal/outcome

  • To regain lost weight
  • To meet the nutritional requirement

Nursing intervention and rationale

  • Assess the patients nutritional status ,to plan for care
  • Record the weight of the patient ,to track the progress
  • Provide a diet in favour of renal cancer,so that the symptoms doesn't aggrevate
  • Plan a nutritional schedule, to avoid unnecessary overload
