M is a 32-year-old man who was referred to MH&A emergency housing by the Assertive Outreach Team. He was living with his girlfriend but the relationship is breaking down and he is at risk of homelessness. He was diagnosed with schizophrenia and had problems with substance misuse. He has diabetes, which was not under control, has HIV and Hepatitis B.
At admission, M. is barely engaging with staff, he is obviously physically and mentally unwell.

And engage the client in some other activities like craft, gardening ect, instead of abusing the substances.

The major problems I identified in the client like,
* he is suffering from human immune virus means acquired immunodeficiency syndrome and hepatitis B, these both disorders are not cure permanently, so these are the major problems.
