He was living with his girlfriend but the relationship is breaking down and he is at risk of homelessness. He was diagnosed with schizophrenia and had problems with substance misuse. He has diabetes, which was not under control, has HIV and Hepatitis B.

At admission, M. is barely engaging with staff, he is obviously physically and mentally unwell.

Mental status assessments and general assessment like head to toe assessments ( physical and mental examination) should be done in order to address to M's issues.

Q. No. 3. Answer :

The client is having mentally and physically unwell, so we need to assess the problems of client, and help him in physical activities and provide emotional support and psychological support to improve his condition.

And engage the client in some other activities like craft, gardening ect, instead of abusing the substances.

And provide psychological and behavioral therapies.

Should not move closely to the client, maintain professional behaviour.

The major problems I identified in the client like,

* he is suffering from human immune virus means acquired immunodeficiency syndrome and hepatitis B, these both disorders are not cure permanently, so these are the major problems.
