Answer is 3.    ......Biological based modalities are example of treatments using natural substances like herbals,food supplementary diets etc for well being of patients. They include foods, vitamins, herbal dietary supplements where paucity of enough Randomised Control Trial data leads to treatment by myth or faith, not by usual FDA approved standard one

Aberration from standard guideline based therapy or treatment such as using massage or drinking chinese tea,(for staying young) accupuncture therapy (say in vertebral disc prolapse instead of standard surgery by a board approved neurosurgeon we are giving pressure therapy) etc are usually termed as complimentary or and alternative modalities. Nutritional supplements e.g using probiotic,herbal medicine etc ( For which we do not have necessary RCT safety test data on animal and then on human volunteers and so on )are also included in the above category. NO 4 ....... in question (Hypnosis etc) falls in the mind body intervention domain of alternative therapy.
