It is so because being in the relationship, it is ethically not right that without the consent of the spouse any of the mutual decision should be taken alone. There should be consent of both the partners for any mutual decision. Although one has right to do anything whatever he/ she may feel like (medically). But ethically there must be mutual benefits for any of the decision taken. Medically the permission is not necessary but ethically there should be a common decision.
