Write an email to your supervisor informing them of a medical error that you made during the treatment of a patient. The attending physician requested a lower PEEP setting than you believed adequate. You chose a higher setting causing distress to your patient.

Respected Sir/Mam ,

With due respect ,I would like to bring into your kind attention that I ( name ) had made a medical error during the treatment of the patient when I was in the night shift yesterday . Physician has ordered to lower the PEEP setting but I chose a higher setting which resulted in respiratory distress to the patient . I have inormed about the error to the physician on duty and resuscitative care was initiated and the patient was made stable . Medical error form is also filled by me yesterday itself and the nurse incharge was also i formed about the same .

I apologise from my heart for what I did and promise that I won't repeat this in future . Kindly, pardon me for what I did .
