I & O is the intake and out put measurements that are monitored for a client. It is very important to record intake and output of fluids in almost all critically ill patients.

This fluid intake is recorded through parenteral administrations,mouth, feeding tubes and intravenous catheters.Out put is recorded through emesis, urine, vomiting, Blood or drainages, perspiration.

There are some nursing actions and care to be followed on such type of clinically ill clients. For the question below my answer is

1. Patient is on I and O and you picked up the lunch tray.

Actually when the patient is on I and O chart, It is very important to measure the fluids strictly with out any missing through out the 24 Hrs. time frame.

So it is very important to calculate the fluids that were taken during, in between and after the meals.So as a Certified Nurse Assistant you must check whether you had carried all the fluids water or juice in the calibrated drinking glasses.
