In these types of situation, it is best to get an explanation from the patient side.This enables the healthcare professionals to identify the cause of their patient's statement. When a nurse asks the first statement ,the chance of patient to explain why her husband says she is a nut case can be riked pit because they often comes out with emotions ,feelings,difficult situation they have gone through.

The second statement is a negative statement by asking about the unsupportive nature of her husband,

The patient is experiencing emotional instability, so a MSE is needed than a pain score
