1.check the patients vital parameters .find out that any bruises or lesions in the body.did he able to walk by himself as he is so week.find out that is he able to walk by himself.is he getting adequate sleep?ask about his concers if he is worried about any thing.check the personel and environmental hygiene.
2.ask him to do walking by himself.check tbat whether he is taking bath ,mouth hygiene.check that how he do his elimination.is he go to toilet or do it in the bed.check his cloth for hygienic .so that we could understand that is he able to do ADL Needs.it is vital to check as he is an old age patient with cardiac disease.and no body is taking care of him in most of the cases.so we need to check that they are getting proper care ,taking proper medications,fulfill their nutritious needs to maintain quality of care and provide a health and happy elderlyhood.
