1. Cleviprex - Calcium Channel Blockers

• Monitor vital signs.
• Monitor for signs of heart failure.
• Monitor liver enzyme levels.
• Monitor kidney function tests.
• Instruct the client not to discontinue the medication
• Instruct the client in how to take the pulse
• Instruct the client to notify the HCP if diziness or fainting occurs.
• Instruct the client not to crush or chew sustained- release tablets.

2. Atropine- Anticholinergics

• Monitor for allergic response
• Assess for risk of injury
• Assess for constipation and urinary retention
• Instruct the client that a burning sensation may occur on instillation
• Instruct the client not to drive or perform, hazardous activities for 24 hours after instillation of the medication unless otherwise directed by the HCP.
• Instruct the client to wear sunglasses until the effects of the medication wear off.
• Instruct the client to notify the HCP if blurring of vision, loss of sight, difficulty breathing, sweating, or flushing occurs.

3. Digoxin - Cardiac glycosides

• Monitor for toxicity as evidenced by anorexia, nausea, vomiting, visual disturbances, confusion, bradycardia, heart block e.tc
• Monitor serum digoxin level, electrolyte level and renal function results.
• Monitor the potassium level, if hypokalemia occurs notify the HCP.
• Instruct the client to avoid over- the counter medications.
• Monitor the client's taking a potassium- losing diuretic or corticosteroids closely for hypokalemia.
• Note that older clients are more sensitive to digoxin toxicity.
• Advice the client to eat foods high in potassium, such as fresh and dried fruits, fruit juices, vegetables and potatoes
• Monitor the apical pulse for one full minute.
• if the apical pulse rate is lower than 60 beats / minute, the medication should be withheld and informed to HCP.
• Teach the client how to take pulse.
• Teach the client the signs and symptoms of toxicity.
• If extreme toxicity occurs, educate to use Digoxin immune Fab (antidote).

4. Mannitol- osmotic diuretic

• Monitor vital signs, weight, urine output, electrolyte levels, lungs and heart sounds for signs of pulmonary edema, signs of dehydration, neurological status, increased intraoccular pressure.
• Assess for signs of decreasing ICP if appropriate.
• Change the client's position slowly to prevent orthostatic hypotension
• Monitor for crystalization before administering the medication. if crystalization occurs do not administer the medication.

5. Propofol - short acting drug that results in loss of consciousness and lack of memory for events

• Monitor for side effects such as irregular heart rate, low blood pressure, burning site of injection, and stopping of breath.
• Note for urine discoloration - green color
• Assess for the decreased intracranial pressure.
• It also induces priapism

6. Versed - Midazolam -

• check for the midazolam infusion syndrome
• check for the symptoms of respiratory depression
• check for drug dependence, addictionand others sedative effect.

7. Calcium gluconate -

• assess for the sideeffects such as nausea, vomiting,decreased appetite, constipation, increased thirst, increased urination etc
• Assess for the toxicity. excess level can also cause kidney stones, deposition in the blood vessels and imapires heart functioning.

8. Fentanyl

• if used in pregnancy assess the fetal heart rate and contraction characteristics
• monitor blood pressure changes
• record the level of pain relief
• monitor for bladder distension and retention

9. Narcan

• same as of Fentanyl

10 . Romazicon - Flumazenil

• Same as Fentanyl
