Vascular Mary is a 64 year-old woman who works as an accountant and has a history of smoking, heart failure and diabetes. Over the past few weeks she has noticed pain in her right lower leg and tingling in her toes after walking for short distances. After a brief rest the tingling and pain goes away. Over the past few days the discomfort lasts anywhere from hours to two days. Just recently her right great toe has developed a black tip that is without sensation. Mary has been admitted to the medical surgical floor for treatment and you are responsible for her care. Mary is allergic to cephalosporins. VS: Temp: 98.1, HR 112 and irregular, BP 160/89, RR: 22, SpO2 89% on room air. Pain 4/10 at rest increasing to 8/10 with activity Current labs Hgb 8.1 Het 29 WBC 18.1 Platelets 209 Home Medications Lisinopril 5 mg po qd Lasix 20 po qd Na +145 K+ 4.5 Ca ++ 8.4 BUN 18 Cr 0.7 Glucose 178 Physical assessment: General appearance: resting comfortably, relaxed and in no acute distress. Weight; 200 lbs, BMI 34 Neuro: GCS 15, AAOx4, weakness and limited in right lower extremity. Cardiac: Skin pink, warm and dry, heart tones irregular, left pedal pulses strong, unable to palpate right pedal and posterior tibial pulses Resp: LS diminished in bases, non-labored, dry hacky cough GI: BS normal, last BM this morning GU: Voiding clear dark amber urine Skin: Skin intact, little hair present on right leg while left leg has normal hair density and distribution. No wound odor is present.
