4)Notice of privacy practice means informing the patients about their rights and maintaining privacy in revealing the health plans of them and the health care provider. It is important because it creates an awareness of the treatment and the health care provider

5)The physical medical record is owned by the physician and original medical record is owned by patient. Mist of the time the provider are the owners of medical records
