Sally Marshall is a newly graduated PMHNP. She worked as a psychiatric nurse for 9 years before going to graduate school. She has been offered a job at the local community mental health center. The clinical manager for the center has told her that her role will consist of seeing mostly adult patients with serious, chronic, and persistent mental illnesses.

The clinical manager also informs Miss Marshall she will occasionally have to see children in additions to adults, when the psychiatrist is “busy.” The manager provides Miss Marshall with further expectations for the job which include; providing medication management for well-known patients and occasionally assisting with diagnostic evaluations of new patients or patients in crisis. He tells Miss Marshall it is understood that she will “from time to time” care for the emergent medical needs of patients who have limited access to primary care providers. She should expect to provide care for patient diagnosed with non-psychiatric disorders, such as, diabetes, hypertension, and chronic pain. Miss Marshall asks for two days to consider the job offer.
