Ans) 1) Inspect the skin under and around medical devices (e.g., tubing, splints, compression stockings) at least twice daily for signs of pressure-related injury on the surrounding tissue. Conduct more frequent assessments in patients vulnerable to fluid shifts and in those exhibiting signs of localized or generalized edema.

2) Nutritional assessment is the interpretation of anthropometric, biochemical (laboratory), clinical and dietary data to determine whether a person or groups of people are well nourished or malnourished (over-nourished or under-nourished). Nutritional assessment can be done using the ABCD methods.
