NURSING INTERVENTIONS
10.1) PRESSURE INJURY-Full skin assesment as soon as possible after admission and thereafter atleast daily or more frequently.Focuss on commom pressure points.Keep the skin clean and dry using unsented skin cleaners that donot cause irritation.Massaging should be avoided.Support the surfaces on both beds and chairs using alternating measures like pressure mattresses.Remobilisation as early as possible.Repositioning should be carried out regularly. During accute attack of asthma semi fowler position can be given depending upon the level of fracture.Ensure adequate supply of nutrients Particularly protein,energy,water and vitamins.
10.2) CONSTIPATION-Hydrate the patient well.Provide high fibre diet.Stool softners and laxatives can be given.Donot ignore the defecation urge
10.3) CONTRACTURES-Change position regularly.Take advice from the physiotherapist on range of motion exercise.Additional pillows should be provided.Provide proper care to the unaffected joints..
10.4)CHEST INFECTION-Timely diagnostic treatment and acute monitoring is required.Proper Diet provided.Encourage to do deep breathing and coughing exercise .Strict aseptic techniques should be followed.Frequent changing of position.
