THERAPEUTIC ACTION- treatment of panic disorder with agoraphobia, social phobia,atypical depression or mixed anxiety disorder and depression, bulimia,and post-traumatic stress disorders as well as borderline personality disorder
EXPECTED PHARMACOLOGIC ACTION- An enzyme called monoamine oxidase is involved in removing the neurotransmitters norepinephrine, serotonin and dopamine from the brain but this drug eases depression by affecting chemical messengers (neurotransmitters) used to communicate between brain cells.
COMPLICATIONS: Dry mouth, Nausea, diarrhea or constipation, Headache, Drowsiness, Insomnia.
Dizziness or light-headedness. Skin reaction at the patch site, Involuntary muscle jerks, Low blood pressure, Reduced sexual desire or difficulty reaching orgasm, Weight gain, Difficulty starting a urine flow, Muscle cramps, Prickling or tingling sensation in the skin (paresthesia).
INTERACTION-the irreversible MAOIs are rapidly absorbed and generally quickly eliminated, with plasma elimination half-lives of 1.5–4 hours during pregnancy or while breast-feeding
certain foods and beverages, avoid foods containing high levels of tyramine ― an amino acid that regulates blood pressure ― such as aged cheeses, sauerkraut, cured meats, draft beer and fermented soy products.
certain medications- other antidepressants, certain pain drugs, certain cold and allergy medications, and some herbal supplements
serotonin syndrome include anxiety, agitation, sweating, confusion, tremors, restlessness, lack of coordination and rapid heart rate
CONTRAINDICATIONS- during pregnancy or when planning pregnancy
children should not be administered
people with suicidal thoughts should be clearly monitored if prescribed.
should not be used with other anti depressants.
MEDICAL ADMINISTRATION: The appropriate dose of MAOI's depends on several factors such as the user's age, health, and several other conditions.
NURSING INTERVENTION: nurses should be vigilant for- sudden onset of a severe, pulsating headache, palpitations, diaphoresis, stiff neck, and nausea
discontinuation syndrome can occur, which causes uncommon withdrawal symptoms such as confusion, detachment from reality (psychosis) and convulsions. you need to make sure that the patient doesn't take them for another two weeks.
CLIENT EDUCATION: should be regarding the diet pattern mentioned above
should be watched for changes in mood, mindset, or attitude unusual behaviour.
when planning pregnancy, it should be talked about to your doctor.
EVALUATION OF MEDICAL EFFECTIVENESS: MAOIs certainly have demonstrated efficacy in the routine treatment of depression. Their efficacy and safety has also been demonstrated in elderly populations
Many psychiatrists are reluctant to prescribe MAOIs for treatment-refractory depression because of concern about having to use a washout period or an unfounded perception that it possess weaker antidepressant properties than newer agents
