In preparing for the first procedure, Janelle, a new hire, asks about a consent form. She is told that the clinic has a blanket consent form for all procedures and the name of the procedure is written on a blank line. Janelle learned about consent forms in school and knows this is not the correct form to be using

1.yes,janelle request for the specific consent form for the COLPOSCOPY EXAMINATION because for all the examination procedure, clinicals having particular consent form in each and every hospital in the world.she can tell to the nurse,the given consent form is incomplete and that's is not a correct one.

2.janelle politely rise their voices for asking the correct consent form doing the colposcopy examination and she should approach the higher authority and request them to perform the necessary changes in the consent form.

3.LIST IN THE CONSENT FORM:

  • Name of the patient for the identification,Age of the patient,she is minor or major,Date,Time and place.
  • Procedure name-colposcopy,procedure description, indication and associated risk-bleeding and infections.
  • Patient declaration.
  • signature of the patients or thumbprint.
  • Name of the health care and signature.
