Here ab was previously taking warfarin.after evaluation he was prescription with valproic acid.these both drugs has protein binding property.due to molecular differences valproic acid can affect action of warfarin when two drugs will be taken simultaneously.

Nursing diagnosis will be

Risk for bleeding related to drug interactions.

For teaching plan ab must be informed about the side effects of valproic acid with the proper dose.he must know how to administer it orally.when valproic acid is taken with warfarin unusual bleeding or bruising can happen.so lab test of PT AND INR should be checked oftenly.

When the patient shares that he wanna take OTC medicine then the nirse should teach him about the drug interactions.
