A 65-year-old man is seen in the preoperative clinic 1 week before a scheduled radical prostatectomy. He has been diagnosed with femoral deep vein thrombosis (DVT) following a complaint of calf soreness.
Question 1) Radical prostatectomy is a surgical procedure performed to remove the entire prostate gland and their surrounding tissue . Which includes seminal vesicles and nearby lymph nodes. It is done to cure prostate cancer in men whose cancer has not spread beyond the prostate.
Question 2) This procedure is performed due to curative reason. It is done to cure prostate cancer in men.
Question 3) Since prostate tumors are very slow growing tumors so there is no urgency although person feel difficulty in passing urine and increased frequency of urine .therefore it is advisable to go for radical prostatectomy since it can spread to other part also
It is a low risk procedure can extent can be 5 years.
Question 4) For treatment of Deep Vein Thrombosis aims
1) preventing the clot from getting bigger
2 ) preventing clot from getting loose and cause pulmonary embolism
For this two goal to achieve we should uae
a) Blood thinner such as heparin . These drug doesn't beak existing clot but it prevent them from getting bigger.
b) clost buster or thrombolytic in serious type of DVT can be given through an I V line or through a catheter directly placed in the clot.
c) compression stockings to prevent swelling from deep vein thrombosis to be worn on leg from feet to knee.
Question 5) These are following risk factors for DVT
Obesity and overwieght
Pregnancy or in first six weeks of post partum
Family history of DVT
Have a catheter placed in a vein
Have a injury to the deep vein
Recent surgery
Women consuming birth c.ontrol pills or relieving hormonal therapy.
Smoking
Being seated for longer duration
Fracture of pelvis ,hip and lower extremity.
