The through puts are any patient who are in critical condition are transferred from outpatient department to inpatient department. then the details should be noted. the output includes the list of inpatient and outpatient department list, patients with specific age group, gender can be clearly viewed.

2. the stakeholders involved are nurses, data entry officer, rt, and physician.

3. the complete review of case sheet and immediate entry of the details related to the disease in the computer and patient care can prevent the problem.

4. the monitor should maintain separate list regarding the inpatient and outpatient entry, and the patients shifted from inpatient to outpatient department.
