Caremore model
Caremore is an integrated care delivery system, and it works by the clinical model, Caremore model. Caremore model is aimed to work for patient education, care coordination , disease management among chronically ill and high risk patients. Caremore model focuses on the extension work of primary health care through Caremore centres.
Comparison Between Geriatric Case Management Assessment and General Assessment in traditional hospital settings:
The act of assessing ( to determine or judge) is called assessment. Health assessment is the evaluation of health status of a person and also it helps in identification of any medical problem. Assessment is an important primary step in all the health care settings. This paper describes the comparison between Assessment in Geriatric care settings and the general assessment in traditional hospital setting.
Geriatrics is a branch of medicine that focuses on health promotion and illness prevention and treatment of disease or disability in later life. Geriatric Case Management assessment is performed to evaluate the functional ability, mental and physical health and their cognition. Assessment in Geriatric setting is also focuses on the evaluation of socio - environmental circumstances of the client. Thus Geriatric assessment is a multi dimensional assessment. The task is performed by number of professionals from different disciplines such as physician, social worker, physical and occupational therapist , nutritionist etc.
General Assessment in traditional hospital settings is done to identify any deviation from normal health and to identify any health problem. Health assessment is mainly done through physical examination and patient's history collection. The task is performed by a physician and a nurse.
Inclusion of nonmedical domains in the assessment process is the main difference of Geriatric Case Management assessment from general assessment in hospital settings. Because Geriatric care management is mainly focusing on quality of life and functional capacity of the client. Physical and psychological approach is needed in the assessment process as psychological and social factors may hinder or mask the symptoms of medical conditions. When comparing with general assessment in hospital settings, more structured approach is needed in the process of assessment in Geriatric settings.
In both settings physical examination and History collection are the two important steps i assessment process , but in Geriatric settings, an extensive review on medications( prescriptions and over the counter drugs), vitamin or other herbal supplements should be done. Assessment of immunization status will be reviewed in both settings. Extensive review on medical and treatment history helps in planning long term care and coordination of care.
Good Survey instruments and well validated tool should be there in Geriatric assessment for checking the hearing, vision, balance, activities of daily living and cognition. Urinary and fecal incontinence is another main problem of Geriatric patients. Thorough assessment of psycho- social environment is necessary in Geriatric or long term care settings as to know the supporting system of the client.
