SITUATION
My name is Mrs. Mary Peter.I'm the primary care nurse caring for Mr. Adam Steve. I would like to speak to you about the patient Mr.Adam Steve. He is showing the signs and symptoms of septic shock.
BACKGROUND
Mr.Steve, a 33-year-old man was admitted yesterday with the diagnosis of septic shock. His father states that he had urinary tract infection for about 2 weeks. He is now showing signs of a fever, dizziness, fainting, cold, clammy, and pale skin. He is in the emergency department.
ASSESSMENT
Mr.Steve's vital signs were, temperature 38^C, heart rate 90 bpm, and respiratory rate 28 b/min. His present blood pressure is 92/40. His WBC at the time of admission was 16.4. Mental status is unchanged. Blood samples are sent for culture and the results are pending.
RECOMMENDATION
I would like to request an order for a chemistry panel, CBC and lactic acid. It would be better if you could visit the patient as soon as possible.
