1. Answer: Prehypertension ( option a)

Prehypertension : Blood pressure readings between 120 to 139 ( systolic) and 80 to89 ( diastolic) mm of Hg is considered as prehypertenson. 128/ 88 can considered as ptehypertension.

Other options:

b)hypertensive : Hypertension is the condition of abnormally high blood pressure ( 140/ 90 m of Hg)

c) hypotensive : Abnormally low BP . 90/ 60 is considered as hypotension.

d) Normal: Normal Blood pressure is 120/ 80mm of Hg

2. Age, Environmental temperature, stress , Exercises .

These Options a, c,d and e is correct. Body temperature can be affected by :

age: Children can have increased metabolism, so the temperature may slightly higher than adults.

Environmental temperature can affect body temperature. It increases as the environmental temperature increases. High environmental temperature is dangerous for the human body as it may cause heat stroke.

Exercise may increase the body temperature, as the metabolism increases during exercise.

Stress also increases body temperature due to various hormones actions.

Nurse should give priority for the pain control. Pai affect the post operative management and healing. it restrict the cient from moving resulting in thrombosis, pneumonia etc.

Other Options are the body's normal response to surgery, slight increase in temperature, decreased blood pressure due to blood loss during surgery etc.  

7. wait for 30 minutes and take the oral temperature ( option d)

Popsicle is frozen item so having grape popsicle affect the temperature of the mouth, may result in error in oral temperature. so nurse can wait for 30 minutes and then she can take the oral temperature.
