The potential of psychedelic medicines for significantly alleviating depression and suffering in relation to terminal illness was demonstrated by psychiatric research in the 1950s and 1960s. Recent studies have shown the safety and effectiveness of psilocybin, MDMA, and ketamine in a medically monitored approach. A single or short set of sessions often leads to significant and continuous improvement in people with treatment-resistant depression and anxiety, including people with severe medical conditions.

Clinicians in palliative care sometimes meet emotional, existential or spiritual patients who persist in spite of optimal therapy. Such suffering can rob people that a life worth living is worthwhile. Data from Oregon show that most people receiving orders are motivated by non-physical suffering intentionally. The article examines the history and therapeutic potential of this class of medicines. Clinical precautions, negative reactions and major steps in relation to safe psychedelic administration are outlined, emphasizing careful patient screening, training, setting up and monitoring.

Even with an increased evidence base confirming safety and benefits, the legitimate use of psychedelics is challenged by political, regulatory and industrial concerns. The federal expanded access program and laws on rights of trial in several States provide for access to medicines which have not yet been granted FDA approval for terminally ill patients. Given the prevalence of persistent pain and increased acceptance of doctor-hurried death as medical response, the legitimate therapeutic use of psychedelics must be revisited.
