In this case atheroma or atheromatous plaque ( lipid deposit) in the coronary artery initially produce a partial occlusion in the coronary artery.

When this atheroma plaque breaks it releases certain chemicals that signal the platelet for altering it function.

Usually the platelet are non adhesive to the endothelium of the vessel, but when the plaque breaks , the platelet get the signal for the adhesion.

When the platelet adhere to the endothelium of the vessel , it iniate another signal fo the platelet - platelet adhesion , it is called the platele aggregation.
