PTSD is a Post-traumatic stress disorder. In order to make a report for the client with PTSD, you should mention the cause of PTSD if patients provide information about the past events like event involving threatened death, sexual violation, serious injury or depression. You need to mention the name, date of birth, place, and age of the patient properly at the top of the report then cause of PTSD followed by symptoms and medication provided to the patients on daily basis.
