I was reviewing the charts of the patients who are coming in today and see that patient A is coming in for complaints of diarrhea; patient B is coming in for a follow up who has ascites; patient C has anorexia and patient D has diverticulosis.

Classmate1: I was reviewing the charts of the patients who are coming in today and see that patient A is coming in for complaints of diarrhea; patient B is coming in for a follow up who has ascites; patient C has anorexia and patient D has diverticulosis.

Myself: Is the loss of fluid under control for patient A? Is this the first time Patient A complaining of diarrhea? If not, is this happening because of IBS?

Does patient B suffer from liver cirrhosis or does he/she have a malignancy in the abdomen or something else? In the earlier visit, was paracentesis required?

For how long has patient C been anorexic? Has the patient visited a psych? how much weight loss as the patient suffered so far?

What has been the treatment history for patient D so far? Would you suggest surgery or will it be cured using antibiotics? Did you confirm the condition using CT imaging or was it path results?
