The cost of healthcare system are expenditure on the health care ( treatment of diseases) and health insurance costs are also a cost of health care.

It means patients insurance status shows whether the patient have taken costly health insurance or cheaper health insurance.

If a person is suffering from dangerous disease, then he will be ready to pay for high premium. On the other hand if a person is suffering from very small disease, then he will not be ready to pay high premium. It means patient insurance status measures health cares costs.

Similarly healthcare spending as a % of the GDP also measures the cost of health care system because it increase the burden of government on the healthcare.

Similarly cost per procedure such as knee or hip replacement also measures the cost of the health care system because it increases burden of the health care.

Patient income level and tax bracket do not measure the cost of the health care system.

Hence option first, fourth and fifth are the correct answer.
