Main aim is to establish effective breathing pattern,regular respiration and no dyspnoea.

  • Assess and record respiratory rate at regular interval.
  • Monitor patient for increasing respiratory distress.
  • Assess breathing pattern.
  • Inspect trachea for deviations.
  • Assess blood gas level.
  • Auscultate breath sounds.
  • Asses weather shorteness of breath is there.
  • Asses the use of accessory muscle for breathing.
  • Assess comfortable position for patient while breathing.
  • Use pulseoxymeter for assessing oxygen saturation and pulse.
  • Note for consciousness level.
  • Assess skin colour,temperature.
  • Provide patient with chest tube and protect the tube and monitor it.
  • Place patient in comfortable position.Avoid side lying position.
  • Administer oxygen if needed.
  • Support and encourage patient for deep breathing.
