Skill : Administration of opthalmic medication
* Description
* It is a non invasivr procedure in which topical instillation of eye medications
into the eye.
* Indicators
* To instill medication before surgery or examination to eyes.
* To treat eye infections.
* For the lubriation of eyes.
* To stain cornea for detect abrasions ans scars.
* Nursing interventions
* Pre procedure
* Prepare the patient for procedure by explaining procedure and give comfortable
position like sitting or lying with head hyperextended.
* Clean eyelids and eye lashes with normal saline and sterile cotton swab
and clean from inner canthus to outer canthus to prevent contaminant enter
into the other eye and lacrimal duct.
* Intra procedure
* Checking ophthalmic medicine for name,dose,number of drops and date
of expiry and instruct patient to look up to the ceiling.
* Placing a thumb of finger expose the lower conjunctival sac and gently draw
down the skin on the cheek.
* Administer eye medication after discarding first drop of in case of liquid medication
or first bead in case of oilment to avoid contamination.
* Post procedure.
* After instillation instruct patient to close the eyelid and not squeeze to avoid
eye injury.
* Instruct the patient to press on nasolacrymal duct for atleast thirty seconds to
prevent medication running down the duct.
* Apply eye pad to reduce risk of injury.
* Outcomes /evaluation
* Assess the response of patient to eye medication.
* Evaluate for any signs of allergic reaction and report physician immediately
if it occur.
* Client education
* Teach the client to wash hands and use sterile water to clean the eye before
procedure.
* Educate to instill medication from inner canthus to outer canthus for avoid
contamination.
* Educate to seek medical care if any complications occur after instillation.
* Potential complications
* Blurried vision.
* Glare or hallows around lights.
* Sensitivity to light.
* Dry eye.
* Nursing interventions
* Eye irrigation to wash out medications from eye.
* Corticostroid therapy to decrease inflammatory responses in eye.
* If complications are due to infection occur from contamination during instillation
begin antibiotic therapy as per doctors order.
* Give psychological suppoert to reduce the stress level of patient related to eye
problem.
