Scenario 1 While working in a medical office, the following three situations occur: A 29-year-old woman has been seen by the physician, who orders application of a Holter monitor. She also needs instruction on the importance of completing the diary. A 62-year-old woman needs an ECG. She is complaining of heaviness in here chest. A 17-year-old patient and his mother have just arrived in the office with written orders from an orthopedic surgeon that he needs a "stat preop ECG." The patient is scheduled for knee surgery in the morning. How would you sort these patients? Who do you see first? Second? Third? Why?

Firstly, I would be choosing the second patient who is 62 year old woman with complaints of heaviness in her chest. An ECG will rule out her heart rate and rhythm, and shows if there is any enlargement of heart due to high blood pressure or for any evidences of myocardial infarction.

Secondly, I would be attending 17 year old patient with a stat  preop order for ECG which is scheduled for knee surgery. A perioperative cardiac evaluation is mandatory to find out the risk for surgery such as uncontrolled heart failures or arrhythmias,or significant calve disease.

Thirdly, I will attend 29 yer old woman who has seen by the physician and came with the orders o application of holter monitor. after the attaching the monitor on her, i will be instructing the needs of completing diary . It is important to document her activites for 24-48 hours of monitoring which will help to diagnose the risk of any arrhythmias.
