As CEO of WeCare, I am happy with what we have done so far advancing patient safety. The Diagnostic Errors concern for us is being managed. Now it is time to look at a second concern and move closer to becoming a High Reliability Organization. The next executive briefing concern that I want to have addressed – Internal Care Coordination. Communication seems to be a big piece of this, so I want us to understand what this means. What do we need to have in place for effective care coordination to prevent patient harm? Before decisions can be made on how to move forward, I need to know what this is, why it is important, and how it can improve patient safety here at WeCare General. Effective care coordination needed to prevent Patient harm..This is the most concern to be more attentive among health care.. communication among providers and Patient at a correct time for correct people can provide safe and it prevent errors with the patient..As a health care providers always focus on high-quality and high value care.. collaboration work with team, care management, pharmacy, technology (HIM) and EHR need sharing knowledge they have to assess Patient needs and plan accordingly, follow up is important to support patient needs that have to link with community resources..
Care coordination among providers and payers improve the outcome and learning and knowledge in primary care practices improve patient care and provider efficiency..focus on patient-centered will strengthen the safety of the Patient and it's practice and system based approach will provide quality and prevent errors among patients..most of the Patient have less knowledge about ttheir health and disease, they don't have idea where to go and approach for the treatment and their appointment.. effective care coordination and communication remove this obstacles to guide them in a right way..EHR improvement made the organization more advanced and it's prevent more of the errors and issues..so right time and right patient will prevent patient harm at we care general..
