patient safety it is the prevention of harm to patients and emphasis is placced on the system of care delivery that 1) prevents errors 2) learns from the errors that do occur and 3) is built on a culture of safety that involves health care professionals, organizations and patients, and patient safety practice is defined as those that reduce the risk of adverse events related to exposure to medical care across a range of diagnoses or conditions, and the practices considered to have sufficient evidence to include in the category of patient safety practices are as , a) appropriate use of prophylaxis to prevent venous thromboembolism in patients at risk b) use of perioperativebetablockers in appropriate patients to prevent perioperative morbidity and mortality c) use of maxmium sterile barriers while placing central intravenous catheters to prevent infections etc.The key principles that serve to motivate healthcare providers to continuously improve efforts in patient safety is that healthcare professionals are intrinsically motivated to improve patient safety because of the ethical foundation, professional norms and expectations of our respective disciplines etc , and to improve safety , healthcare professionals must recognize the characteristics that can make this efforts a success, first we must be able to collect the data on error and incidents within the local organizations in order to identify opportunities for improvements and to be able to track progress and second is to develop an organizational culture that is founded on the concept of safety and third is that complex processes must be analyzed using appropriate tools and finally as much standardizations as is possible should be accomplished while still allowing individuals the independent authority to solveencountered problems in a creative way.
