Post voiding catheterization yielded 100 mL and the urinalysis showed E coli>100,000+ HPF. RBC 3-6 bacteria rare. The urinary culture and sensitivity results are as follows: E coli > 100,000 colonies, sensitive to ciprofloxacin, trimethoprim/sulfamethoxazole, and nitrofuradantin. 1. State the abnormal lab values and what they could represent. The medical director makes rounds and writes to start an IV of D5 in.25 normal saline a 75 ml/hr, and insert a retention catheter (Foley catheter) to straight drainage. Because Ms. Zippo is unable to take oral medications, the physician ordered ciprofloxacin 200mg, bid IVPB. 2. Is the type and rate of fluid appropriate for Ms. Zippo's age and condition? 3. 1. Zippo has elevated WBCs, Urinalysis shows the presence of RBC and bacteria. Urine culture shows the presence of E.coli. These show that the patient has been infected with Urinary tract infection.

2. Post voiding catheterization yield only 100mL. The IV Flow rate is too high for her age which leads to overhydration. The organism E.coli is sensitive to ciprofloxacin. Hence the physician order for ciprofloxacin medication is not appropriate for this patient.

3. Ms.Zippo's has looked for the symptoms of

• leaking of IV fluid
• Local edema
• erythema
• Pain
• skin blanching
• Shortness of breath in case of fluid overload.

3. The organism E.Coli causes urinary tract infection in this patient. E.coli is commonly present in the GI tract that transmit into the kidney and cause infection into the bladder and urethra.

4. The patient already has dehydration. Applying physical restraints for her age cause skin injuries.
