As a new patient calls to schedule an appointment and there will be no information about the patient we should collect all the information about the patient.

Steps.

First I will sign the patient as new to the facility.

Ask her full name.

Telephone number.

Date of birth.

Gender.

Reason for the appointment or complaint of the health.

Type of insurance.

Insurance number.

Social security number.

Address.

Emergency contact and their information.

Refering physician.

And I would double check to know that I take necessary information and explain about her appointment date an time.
