Constipation is defined as the difficult or the incomplete passage of stool as excessive hard and dry state may be painful sometimes to pass. The chronic constipation may result in hemorrhoids, ulcers as well as the colon cancers.

The reasons for such constipation may be low intake of fluid specially drinking water, limited fiber rich diet, the person is on certain medication, stress, tumor as well as certain type of neurogenic disorder.

As per the nursing assessment and intervention is concerned in the care plan of the constipation there is need to have a careful history of the patient along with the physical as well as the radiological diagnosis to specify the problem.

There is need to check the pattern or the consistency of the stool passed.

Then there is need to check the patient’s eating habits, schedule and the fluid intake.

Then there is need to assess the level of the activity of the patient as it also affect the passage of the fluid as it is related to the gut motility as the peristaltic movement and normal gastric emptying.

Then asses the type of the medication on which the patient is running may be that is the cause of the constipation.

Check if there is any sign of the fear or the stress in his/her life as well as the history of any neurological disease.

Then encourage the patient to enhance the fluid intake if required use enema as well as the suppositories.

Advise patient to use some fibrous food and increase the physical activity to assure the bowel movement.

Thus by the proper care plan the cases of constipation can be handled in a effective manner.
