There are six QSEN competency which can be used by the nurse to provide the best possible care. They are

1. Patient centered care focuses on the needs if the patient and planning care accordingly
2. Improving the quality of services renderd to the patient
3. Creating a safe environment for patient in protecting confidentiality, patient privacy,fall prevention,prevention of medication error,etc.,
4. Always working in a team and exchanging information to continue the care by subsequent staff
5. Practicing on the grounds of evidence based research
6. Use of information technology for faster exchange of data and provide timely care

●The nurse providing handover is a teamwork and collaboration QSEN competency behaviour

●The highest admission rate can be easily identified from the patient admission records

Identifying what type of illness or diagnosis the patients are admitted frequently and treated for

Checking under which department or speciality tte patient is admitted gives a clear cut knowledge on admission rate.

●Some of the steps which could have been done for improving self manages post discharge are

• Health education on the disease process
• Take medication on time do not skip or overdose
• Check tte patient thoroughly before the time of discharge
• Provide education in diet and medication

●Readmission within 30 days for the same complaint means that there is a issue on the quality of care delivered.

●The nurse can do the following

• Health educate
• Screen the patient under high risk for readmission while discharge
• Provide the best quality care during the index period and observe for any long term complications and to take appropriate preventive plans

●The QSEN behavior suggests for a quality improvement to reduce the readmission

●The nurse could respond by asking the patient preferred choice of food and making arrangements so that the patient can eat it.
